Do I need a will or durable POA

More details to this question:
I have medical and general POA for my best friend who's health is declining rapidly (currently ill in a nursing home). I already handle his finances but not real estate. What do I need from him to carry on with his real estate issue, even if he passes?

Powers of attorney lose all legal effect when the person granting them dies. Wills govern property after death but have no legal effect until a court admits them to probate (proving).
