Estate, living trust, final expenses,

More details to this question:
I need information regarding my estate. leaving it to my wife all the contents ( Home, Cars ect.) I have health condition I may need someone to make decision for me ? If something happens to both my wife and myself what do I need to have in place ? Final expenses ? A Will who gets what ?
