How did my spouse get my name off the deed without my knowledge?

More details to this question:
I learned during the divorce process that he filed, that my name wasn't on the deed giving him total control over home sale decisions. We both signed that deed! Only HE is on the mortgage. I'm devastated because that's home equity I need to live on. How can I prove I did sign that deed? Did he do something illegal?
