How do I receive conservativeship for my 28 year old daughter?

More details to this question:
She is on disability,but does not have control of her own money because of drinking and drugs. She still manages to get them,especially Meth. She's been in detox 4 times this year. They always send her back home. No good .She needs rehab for more than a week or 2.She won't really go on her own. It's killing her.please help.

You file a petition in court asking to be appointed as her conservator. See the court clerk in the county where she lives.
