How to fill out form PC-442A: Proposed distribution final

More details to this question:
I am the fiduciary for my 18 year old daughter's account. She just turned 18 and I need to complete forms PC 442, PC 442A & PC 412. I am unsure how to fill out the itemized list. Does the full bank account get distributed to her (front of page)? There are no other expenses or properties.
