More details to this question:
I have two Aunts 88 & 91. The three of us want to make sure they have me to make medical decisions for each of them in case one or both become ill and require medical decisions. Can we do this with a notarized letter or do we have to go through a will?

You need to have a Medical Power of Attorney drawn up for each one of them. A will is not needed. A will is to dispose of property after death. Hope they both have capacity so they will be able to sign the documents knowing what they are signing.
