marriage certificate and birth certificate do not match

More details to this question:
The legal name born is John Malcolm Smith. John marries 2 times as John M. Smith. Has 3 children as John M. Smith. Passport, Deeds, records all show John M. Smith. John some how gets a Nevada Drivers License that only shows John Smith (middle name is missing). He gets married a third time using the inaccurate Nevada Drivers License as John Smith --- he never changed his name. Is the marriage valid, as the I.D. is not correct and not the same as the Birth Certificate?

Based on the facts that you have presented, the marriage is valid.  Middle names are not consequential in this regard.
