Survivorship Form help

Husband is not in good health. Has had stint for abdominal aortic aneurism, shoulder surgery for rotator cuff, and told 2 years ago he had only 21% lung capacity. First we were told looking at lung transplant in 2-5 years but a new procedure was approved that was non invasive. Most recent CT Scan showed a new and fairly good size spot in his lungs, considering that it just appeared in past 3 months. Also, unlike the ones theyโ€™ve been watching, this one has spider legs. We/I am terrified itโ€™s cancer. We have nothing taken care of in event of tragedy. I got survivorship paperwork but have no idea how to fill them out. But I also have had health issues and havenโ€™t held a job where taxes were taken out in about 5 years. Was a stay at home mom for many years. We/I donโ€™t want to worry about court battles for our home, belongings or bank accounts if the worst possible things that could happen, do. Iโ€™m pretty good at figuring things out. But at this time my thoughts are clouded with fear.

I'm sorry for the situation you find yourself in.  An estate planning attorney can easily guide you through the process.  With all that you're dealing with it's certainly understandable to be a little confused.  Depending on what is needed you may have little to no out of pocket costs even from a private attorney.
