Taking emergency custody of my father

How can I get emergency guardianship of a parent that is not coherent. HIs girlfriend has a medical power of attorney that we know is forged. She does not seem to be acting in his best interests. We are also aware that she is currently attempting to forge the transfer of title of his house and cars. What steps can I take to step in and get emergency guardianship.
