Who Files Order? Termination of Counsel

More details to this question:
The client/lawyer relationship just isn't working out. My lawyer doesn't communicate, documents presented haven't been professional, he is always out of town and refused to meet with me. Do I file the termination letter with the court or is the lawyer required to submit an order?

You can either get another lawyer and have that lawyer file for a substitution or you can ask the lawyer you have now to file a motion to withdraw.
