How do I find the correct form to file inventory for my mother?

More details to this question:
I was recently appointed as conservator for my mother and the court wants an inventory filed. I have yet to find exactly what the judge is looking for. This is in Phelps county and I currently ( as well as she now) reside in St. louis county.

The lawyer who represented you in applying for conservatorship should be able to advise you on this and file the properly completed Inventory for you.
