If I am put on medical leave due to workman's comp does this fall under personal leave and FMLA?

More details to this question:
I was injured on the job on 04/08/14 and am having to fight with my company over workman's comp. I have not worked since 04/09/14 as the dr I saw refused to sign my return to work as he did not feel qualified to make that decision & wanted me seen by a specialist, which my company will not guarantee payment if I do. I received a Personal Leave packet from my company but I never requested a personal leave, I would go back to work under restrictions if the dr would approve it. If i fill this out it states that my job is not guaranteed and I will not be paid for my time off. This packet must be filled out & returned by 5/19 including a section from the dr. I spoke with the Leave Dept and they said that though this is under personal leave it won't apply to me because I'm workman's comp. I'm nervous to fill this paperwork out for fear of them coming back stating I signed off on something I shouldn't have. But if I don't fill it out I'm not complying to their demands & could be terminated
