workers comp question

More details to this question:
an employee at a nursing home is attempting to obtain the workers comp for that the employer completed for her. The employee has signed a release of information request for that office, however they are refusing to provide it. The employer altered the portion the employee filled out and does not want to provide this information. They also obligated the employee to go to a certain clinic, instead of allowing her to use her primary doctor. Is any of this legal? Please advise. Thanks!

You are required to sign a release of medical information.  I am not sure who is refusing to provide that information.  The doctor's office must provide copies of the records.  The employee should fill out the information form.  We do this for our clients.  Please feel free to call us at 860-447-3171.  Attorney Ralph Monaco
