If it was 50% FEES this is not appropriate without a court order/approval. If its 50% with FEES and COSTS combined, this is perfectly normal as the costs of the case are IN ADDITION to the fees, generally. Medical bills incurred are paid out of your portion AFTER FEES AND COSTS.

I cannot think of any reason why an attorney would not take the case based on the facts that you are conveying, but perhaps you do not know all of the pertinent facts.
