First, congratulations on your recovery.
At some point, you will have to go to court to assert your rights. Strategically, when you have been clean for a year, you can show the court that you have taken responsibility and are now doing all the right things to rebuild your life.
When you have affidavits from your caregivers and recovery partners, that will look strong for your case. Start with getting visitation and then you can build to custody.
For now, be patient but I would start to build your case with the ex that you really have changed. Tell him what your plan is, demonstrate you proving your commitment to that plan and he might start feeling different. You have to help him overcome his anger.'

This is a tough situation. First, if your girlfriend has a legal custody agreement with the children's father, that agreement would likely spell out who obtains legal and physical custody if one of them passes away.
She can certainly appoint a guardian for her children in a legal document, so long as any custody agreement doesn't prevent this. Divorce is hard on everyone involved. Assuming everything you say is true about the mom, unless there are drugs or alcohol or abuse involved, court's will do everything they can to keep the mother in the picture because that is usually deemed in the best interests of the child.
At some point, if she is unable to peform at all as a parent, the court might entertain terminating her parental rights but, courts will only do so with very good reason.
Having seen a lot of divorce matters over the years, you don't have to like her but, for the sake of the child, remember that they will do better if she does better. The child deserves it.

You can certainly ask for full custody. However, it will be very expensive litigation battle.
